“Eye contour surgery” is a procedure that can sound unfamiliar even to people who are very interested in plastic surgery.

That is because when people hear “contour surgery,” they usually think of procedures that reshape the facial bones to create a more balanced and beautiful facial contour, such as square jaw reduction, cheekbone reduction, V-line surgery, or double-jaw surgery.

In Korea today, in addition to the basic and most commonly performed double-eyelid surgery, there are various eye procedures such as ptosis correction, epicanthoplasty, lateral canthoplasty, and lower canthoplasty. With advances in eye surgery, however, it is now possible to perform contour surgery on the eyes, much like facial contouring surgery. This procedure changes the contour of the eyes by adjusting how much of the iris and the upper white of the eye are visible.

Ptosis correction is a surgery that increases the vertical width of sleepy-looking eyes caused by drooping eyelids (ptosis). Recently, as circle lenses make the iris appear larger, the number of people undergoing ptosis correction has increased significantly.

Epicanthoplasty helps correct the appearance of wide-set eyes and gives the eyes a more open, refreshed look. Recently, as epicanthoplasty techniques that leave almost no visible scarring have been developed, it has become as common a type of eye surgery as double-eyelid surgery.

Lateral canthoplasty involves making an incision at part of the outer corner of the eye to make the horizontal length of the eyes appear longer. However, it has the drawback of being less effective when the eyes are deeply set. To overcome this limitation of lateral canthoplasty, lower canthoplasty is performed together.

Lower canthoplasty is a relatively recent surgical technique with a high level of difficulty, and it is less widely known than epicanthoplasty or lateral canthoplasty. It is also called lower eyelid S-line surgery. By lowering the outer part of the lower eyelid downward, it softens eyes that give a sharp impression and creates a more open, gentle-looking eye shape. It can be applied when the outer corners of the eyes are upturned, when the pupils appear pulled downward, when the eyes are small, or when the eyes look sharp. Because lower canthoplasty is performed through the inner conjunctiva of the lower eyelid, it does not leave a visible scar.

It is now possible to change the overall contour of the eyes through ptosis correction, epicanthoplasty, lateral canthoplasty, and lower canthoplasty. As a result, creating the eye shape best suited to each individual has become possible.

Director Hong Seung of Hyundai Aesthetic Surgery explained, “With the development of eye surgery, it is now possible to change the overall contour of the eyes, so eye surgery should also be approached with the concept of adjusting the eye contour.”

He added, “To create the natural impression and eye shape that are the most important points in eye surgery, procedures should not be performed indiscriminately. Instead, the design and surgical method should be chosen through sufficient consultation with a specialist, based on one’s overall facial shape and eye shape.”
