Lowering a Double-Eyelid Line to Improve an Artificial-Looking Eye Shape

Double-eyelid surgery is one of the most preferred cosmetic procedures, and many patients have chosen and undergone it to improve the shape and impression of their eyes. However, there are cases where the result after double-eyelid surgery is unsatisfactory or side effects occur, making revision surgery necessary. As in the surgical case above, this can happen when the line after double-eyelid surgery is excessively high or formed in an artificial-looking way. A large proportion of patients also contact me in person to improve this issue. As a way to resolve thick, artificial-looking eyes, lowering the double-eyelid line can be performed. Through this surgical method, I was able to naturally lower a thick and high double-eyelid line.

As in the surgical case above, lowering the double-eyelid line is a process of adjusting the height of the existing double eyelid to make the eyes look more natural and harmonious. During consultation, we consider each individual’s eye shape and proportions, identify the patient’s needs, and adjust the line to an appropriate height before performing the surgery. The surgical method may vary depending on the amount of excess skin. If there is sufficient excess eyelid skin, improvement can be made cleanly by excising the scar and eyelid skin together. However, if there is insufficient excess skin, it can be improved through the two-line fixation release method. The concept and method of two-line fixation release may feel somewhat unfamiliar. This method does not excise the existing skin; instead, it releases the existing high line and creates a new line to lower the double-eyelid line.

Because it releases existing adhesions, scars, and unwanted lines, it has the advantage of allowing a new line to be set symmetrically. After double-eyelid line lowering surgery, traces of the previous double eyelid gradually fade over time. In the case above, which I personally operated on, the line was improved using the two-line fixation release method. When the eyes are closed, traces of the previous double eyelid may be slightly visible. However, the existing scar and adhesions were released, and since this is only 14 days after surgery, the previous scar is expected to gradually become lighter over time.

Surgery to improve a thick and high line is inevitably a highly difficult procedure in many respects, because it must involve excising the double-eyelid tissue or releasing adhesions while simultaneously forming a new line. Therefore, it can be said to require even more delicate technique and concentration. However, for double-eyelid line lowering surgery, it is important to determine the appropriate timing by considering the patient’s skin condition and recovery speed.

In general, tissues gradually stabilize 6 months to 1 year after double-eyelid surgery, and I tend to recommend surgery during that period. However, this does not apply to every patient, because each patient has different skin characteristics, eye conditions, and previous surgical history. We assess these factors and determine the timing of revision surgery accordingly. To prevent repeated revision surgeries, sufficient consultation with a specialist is also necessary.
